Description

The RN Care Specialist communicates and collaborates closely with physicians and other members of the care team to provide consistent quality care for patients. Working with a high degree of autonomy, the RN Care Specialist accepts responsibility and accountability for assigned patients as a clinic nurse and care manager to plan implement and evaluate the care for patients. The RN Care Specialist assists patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes. This key position will be required to work within a collaborative care team through development of all aspects of clinical workflow and care management operations within the medical home model.

Required qualifications:

• Graduate of an accredited school of nursing.

• Upon hire: Montana Registered Nurse License

Preferred qualifications:

• Bachelor's Degree in Nursing

• 2 years of Clinic Nursing

• 2 years of Nursing

• Medical home care model
